The River Vale Board of Education requires that all students entering Pre-school, Kindergarten and Grade 6 at Holdrum Middle School provide an immunization record and evidence that they have had a physical examination by their pediatrician. This physical exam must have been performed within 1 year of the first day of attendance at school.

Pre-Participation Physical Evaluation Form is the ONLY form accepted at Holdrum Middle School.
All students interested in participating in sports or intra-murals at Holdrum must submit this form.
All physical exam forms must be complete, signed and dated by the physician, parent and student where indicated. Incomplete forms will not be accepted.
If you are unable to provide your child with this physical exam, the school physician may be contacted by the School Nurse to perform this examination.
The River Vale Board of Education recommends that parents provide evidence that their child has received appropriate dental care. When your child visits the dentist, please have the Dental Form completed and return it to the Health Office.
